Lakeside ENT & Allergy is pleased to announce the grand opening of our third practice location in Batavia. Located at 215 Summit St, the new office is across the street from UMMC. Lakeside ENT & Allergy adds a GE ultrasound to our main CDGA office to meet the needs of our patient population.  Because we provide care to a large number of patient with thyroid diseases, the US will allow us to perform point of service diagnostic imaging and US guided biopsies of thyroid tumors, goiters, and other neck masses

In some cases your insurance company may require you to get a referral from your primary care physician so that you can consult with an ENT physician. Your insurance company may also require you to pay one or more small co-payments at the time you receive the health service in one of our offices.

What is a Co-Payment?
Under the terms of your contract with your insurance company, you may be responsible for a co-payment. That means that each time you visit a health care provider's office, you must pay one or more fees. If you aren't prepared to pay the fee(s) as required by your contract with your insurance company, you may not be able to receive medical care on that day.

Co-payments are required for services performed by all health care professionals, including nurse practitioners and audiologists. If, for example, you see the doctor and also have hearing testing on the same day, your insurance company may require you to pay two co-payments, one for the doctor and one for the audiologist.

What is a Referral?
If your insurance plan requires a referral, that means your primary care physician (PCP) must authorize and approve your visit to our office. Once your primary care physician determines that you need to consult with an ENT specialist, your physician will contact your plan to request approval in the form of a referral. Your referral must be authorized and approved by your insurance company BEFORE you visit our offices or you may be expected to pay for the office visit.

You are responsible for knowing whether you need a referral from your insurance company and whether you have received one.

You can check the status of a referral by checking with us prior to your appointment, or by calling Member Services department at your insurance company.
