LAKESIDE ENT - INSURANCE
You may have contracted with a health insurance company
which has agreed to help pay the costs of your medical care. In
exchange for paying your health care expenses, your insurance company
may require you to get a referral from your primary care physician
so that you can consult with an ENT physician. Your insurance company
may also require you to pay one or more small co-payments at the
time you receive the health service in one of our offices.
What is a Co-Payment?
Under the terms of your contract with your insurance company, you
may be responsible for a co-payment. That means that each time you
visit a health care provider's office, you must pay one or more
fees, ranging from $5 to $20. If you aren't prepared to pay the
fee(s) as required by your contract with your insurance company,
you may not be able to receive medical care on that day.
Co-payments are required for services performed by
all health care professionals, including nurse practitioners and
audiologists. If, for example, you see the doctor and also have
hearing testing on the same day, your insurance company may require
you to pay two co-payments, one for the doctor and one for the audiologist.
