To Make an Appointment
Call our offices Monday through Friday, 8:30 a.m. - 5:00 p.m.
To Cancel an Appointment
We strongly urge you to call our offices at least 24 hours in advance
if you can't keep an appointment. Everyone will appreciate your
courtesy, and another patient will have the opportunity for an appointment.
This will also ensure that your medical care is continued appropriately.
Things to Remember When Visiting Our Offices
- Referral
- Before you arrive, make sure your referral,
if needed, is current.
- Insurance Card
- Bring your insurance
cards every visit. We copy them and use the information to bill
the insurance company directly. Insurance plans are frequently
updated.
- Copayment
- Bring your copayment(s)
if your insurance company requires them.
Payment
We require payment, including copayments, at the time of service,
unless other arrangements have been made in advance. We accept Master
Card, Visa and Debit cards. If you have no insurance, you are expected
to pay the fees in full at the time of the visit, and prior to surgery
should you need it. You will also have to arrange to pay for hospital
costs.
Prescriptions
Call for prescription refills during office hours, 8:30 a.m. - 5:00
p.m., Monday through Friday. That will allow us to review your chart.
Call well before you have run out since some prescriptions may not
be able to be filled on the same day. It will speed things along
if you have your pharmacy phone number, your name, and (in the case
of a child) weight handy when you call.
Emergencies
For serious problems, such as uncontrolled bleeding or difficulty
breathing, call 911 and get to the nearest emergency room. The hospital
will contact your physician.
For other problems, it is best to call your doctor
during office hours. Most medical problems can be taken care of
quickly during the day rather than evening or night.
If you need to speak with a physician after office
hours, call our offices to reach our answering service. Our service
will contact the physician on call, who will call you. A physician
is always available to help you.
Transferring Your Medical Records
We handle all medical records with strictest confidence. We require
your written permission and signature before we can send information
to anyone. Please allow two weeks for completing the paperwork.
