Referral – Before you arrive make sure any necessary referrals are current.

Insurance Card – Bring your insurance cards to every visit. We copy them and use the information to bill the insurance company directly. Insurance plans are frequently updated. Please note that as of 1/1/2024 we no longer accept Fidelis Medicaid or United Health Care Community Plan for New Patients.

Copayment, Co-Insurance, and Deductible – Payment is expected in full at the time of service or per the terms of your insurance plan(s) unless prior arrangements have been made.

No Surprises Act - When you get emergency care or get treated by an out-of-network provider at an in-network Hospital or Ambulatory Surgical Center, you are protected from surprise billing or balance billing.

Allergy testing - for patients scheduled for allergy skin testing please refer to our allergy testing page for important information.

Missed Appointments, No-Shows, and Cancellations - We require 24-hour prior notice for cancellation of office appointments. Cancellations without 24-hour notice will be charged $50.00. We require 7-day prior notice for cancellation of surgical procedures (Hospital or Office). Cancellations without 7-day prior notice will be charged $150.

All missed appointments without prior cancellation are considered “no-show”. Patients who consistently no-show to appointments are at risk of being limited to emergency care only or dismissed from the Practice.
